Composition Selection and Payment Agreement Form

  • I am selecting the following composition type for this event/recipient and agree to the following terms:
  • Reminder: A deposit of half of the price is due at the time of booking for all composition requests. Composition requests, up to six instruments, must be booked 1 month prior to the event for editing purposes. If an event is booked less than 30 days from the time of the event, the buyer acknowledges that editing opportunities/input will be forfeited and the payment is due in full at the time of the purchase. Longer /Other composition requests will be booked/accepted based on timing of the event and agreed upon between the client and Musical Monograms, LLC.
  • Deadlines 

    - Submission Date: All forms must be completed and processed in good order
    - Acceptance Date: Once forms are received and down payment is made and processed

    Buyer will receive a confirmation email of acceptance for the composition request. Once this email has been sent,
    the buyer has 3 days to remit the down payment.


    -Edit input date is 2 weeks from the acceptance date. Longer /Other composition requests will be booked/accepted based on timing of the event.

    Editing input Deadline 1: Buyer will be sent an initial sample of composition (any form or part of lyrics, melody, theme, etc.) 2 weeks after booking for works up to six instruments. Longer /Other composition requests will be booked/accepted based on timing of the event. This gives the buyer the opportunity to give feedback or share any information concerning the composition. Continuation of the composition will not be delayed due to lack of buyer’s response. Final production is at the full discretion of the composer(s).

    -Final production date and Final payment (2 weeks prior to event)

    Final Payment Date: All remaining balances must be paid two weeks prior to the event date in order to receive a copy of the composition and a Signature Keepsake Music Box of honor. Once the remaining 50% payment has been made, a copy of the track/recording will be sent to the buyer 1 week prior to the event for works up to six instruments. As seen below sound recordings: Accompaniment tracks (no vocal) may be provided 1 week prior to the event deadline upon request (please see the event form). All recordings will be delivered to the client/buyer unless otherwise indicted on the event form. If client chooses to have a sound recording delivered directly to the recipient, it must be indicated on the event form. Once any form of final composition has been sent to the client and or the recipient, no refund is available on the final payment.

    Distribution of final composition 

    Final distribution included a sound recording (digital and CD) will be sent to the client/buyer. The information on the client form will be used to deliver the sound recording. A Signature Keepsake Music Box will be mailed to the recipient unless otherwise indicated on this form. For elements of surprise, the client/buyer may choose to have the Signature Keepsake Music Box mailed to his/her address provided. The default is for the Signature Keepsake Music Box to be mailed to the recipient’s address provided on the client form.

    -Signature Keepsake Music Box

    Recipient will receive (unless otherwise noted) a Signature Keepsake Music Box with the title of the original work purchased in their honor. The Signature Keepsake Music Box will include the month, day and year of the event indicated on the client and event form. The name of the recipient will be listed on the Signature Keepsake Music Box as well as the copyright and rights reserved to Musical Monograms, LLC.
  • -Sound Recordings

    Client/Buyer will receive a sound recording sent to the address and email indicated on the client/buyer information provided on the client form prior to the event deadline. Songs (involving lyrics) will include a lyric sheet. Accompaniment tracks (no vocal) may be provided 1 week prior to the event deadline upon request (please see the event form). All recordings will be delivered to the client/buyer unless otherwise indicted on the event form. If client chooses to have a sound recording (in addition to the Signature Keepsake Music Box) delivered to the recipient, it must be indicated on the event form.
  • Once any form of final composition has been sent to the client and or the recipient, no refund is available on the final payment
