  • The agreement below is binding between two parties: Musical Monograms, LLC and buyer.
    Client/Buyer Information (all requests must be made by individuals 18 years or older).

  • I acknowledge that a deposit of half of the price is due at the time of booking. Composition requests, up to six instruments, must be booked 1 month prior to the event for editing purposes. If an event is booked less than 30 days from the time of the event, the buyer acknowledges that editing opportunities/input will be forfeited and the payment is due in full at the time of the purchase. Longer/Other composition requests will be booked/accepted based on timing of the event.
    A percentage of every purchase is donated to St. Judes Hospital.
  • RECIPIENT INFORMATION
  • A Signature Keepsake Music Box from Musical Monogram, LLC with information including song title and recipient name will be sent to the above address that includes a copy of the composition unless otherwise indicated on this form. Please indicate if you would like for the Signature Keepsake Music Box from Musical Monogram to be sent to the buyers address. If no indication is made the default is for the Signature Keepsake Music Box to be sent to the recipient and not the buyer.
